How can I complete the questionnaires?

Completing the questionnaires is very simple. The questions are all easy to understand, and are answered with Yes or No or Maybe. Follow your initial feelings – don’t spend a long time thinking about the answers. Each questionnaire will take approx. 15 minutes to complete.
